Expansive epilepsy networks: Can we target common nodes before the damage is done?

Summary: Focal lesions can cause secondarily generalized seizures, through local, regional, and then global propagation of epileptic activity. Another pathophysiologic mechanism -over time- is via projection onto existing healthy brain networks, rendering them dysfunctional and even epileptogenic. In this model of “secondary network epilepsy”, the original lesion may still drive the seizures, but the focus is masked by synchronized diffuse EEG activity. Now, the seizures are no longer distinguishable from non-lesional etiologies. In this presentation we will review recent insights in focal epileptogenesis, its progression towards diffuse epileptic encephalopathy, and the potential for network-based treatment targets.
